Individuals who have not applied for a Multimedia Center card or National Central Library card and are at least 6 years old may apply online. (Please fill out the application form at the counter for lost and replaced cards.)
備註：多媒創意實驗中心為6歲以上民眾可入館，總館為16歲以上民眾或未滿16歲之高中職或具同等學力之在學學生可入館。
Note：Admission to the Multimedia Center is open to people over 6 years of age ; admission to the National Central Library is open to Individuals over 16 years of age or to high school students under 16 years of age or to students with equivalent qualifications.

When applying for a library card, one needs to follow the “Library Regulations on Reading Services” and other related rules and regulations.
- 填寫完畢後，請在三個月內持身分證件，於辦證服務時間內親洽本中心辦證櫃臺領取RFID借閱證或智慧閱覽證（請自備悠遊卡或一卡通）。超過三個月未領借閱證者請重新填寫申請表。
Getting a card：After completing the application for a library card via the Internet, the applicant needs to visit the Registration Desk during office hours to obtain your library card or a smart library card（the applicant needs to have an Easy Card or IPASS Card to obtain this card）within three months of the application date; the applicant must have a valid ID card or certificate. If the applicant fails to pick up the card within three months of the application date, he/she must begin the application process anew to obtain a library card. Applications for a Smart Library Card commenced on March 1, 2018.（An applicant must have an Easy Card or IPASS Card to obtain one.）

多媒體創意實驗中心(以下簡稱「本中心」)謹遵個人資料保護法相關規定，對於使用者因辦理本中心借閱證，而由本中心取得之個人資料或各項個人借閱流通使用紀錄，必將依法善盡保密之責：
The Open Lab (hereafter the Library) discreetly follows all relevant rules of the Personal Information Protection Act to meet its responsibility for preserving the confidentiality of any personal information or individual circulation records of the applicant acquired by the Library by applying for a library card.
一、蒐集範圍：
Scope of personal information gathered by the Library:
(一)本中心於使用者申辦借閱證，提供圖書調閱、借閱及空間設備登記使用等服務時，將蒐集、處理或利用您的姓名、生日、身分證字號、護照號碼、教育、職業、電話、電子郵件信箱、通訊地址、戶籍地址等個人資料。
The Library will collect, arrange, or use the applicant’s personal information, including the name, date of birth, photo, ID number, passport number, education, occupation, telephone number, e-mail address, correspondence address, permanent address, etc. obtained when applying for a library card or when the Library offers services, such as transferring library materials from another library.
(二)當您使用本中心提供之各項網路資源服務，本中心及資料庫廠商將使用cookies 進行管理及記錄。包括記錄IP位址、瀏覽器種類、使用檔案及時間等軌跡資料。
When you use any internet resource services offered by the Library, the Library and the database contractor use cookies to manage and record the services rendered, including the trajectory data such as recording the IP address, the kind of browser, the files used, the length of time, etc.
二、蒐集之個人資料類別
The various types of personal information gathered by the Library
C001辨識個人者：姓名、電話、電子信箱、戶籍及通訊地址。
C001 Personal recognition: name, photo, telephone number, e-mail address, permanent address, and contact address.
C003政府資料中之辨識者：身分證字號、護照號碼。
C003 Recognition in governmental documents: ID number and Passport number.
C011個人描述：性別、生日、國別。
C011 Personal description: gender, date of birth, and nationality.
C038職業。
C038 Profession.
C052資格或技術：學歷。
C052 Qualifications or skills: educational background.
三、蒐集目的：
Purposes for collecting personal information:
本中心蒐集使用者資料之目的，係為明確使用者使用本中心各項館藏資源及空間設備之權限，或執行館藏資源流通業務而為之。
The purposes for collecting personal information of library patrons are to ensure the rights of its patrons to have access to any and all of the Library’s resources and to enable the Library to execute the business of circulating its resources.
四、讀者權益：

基於我國個人資料保護法之規定，您可行使下述權利：
According to the regulations of the Personal Information Protection Act of Taiwan, the following rights of all library patrons include:
(一)請求查詢本人之個人資料。

(二)請求更正本人之個人資料。
Request to change one’s own personal information
五、您可選擇是否提供本館您的個人資料，惟您不同意時，本館則無法提供各項閱覽服務。
A library patron can choose to offer his/her personal information to the Library. If he/she chooses not to, the Library cannot offer such patron its reading services.
